What are the responsibilities and job description for the Medical Case Manager Specialist position at Interim HealthCare Home Health and Hospice?

You'll be responsible for examining patients, conducting assessments, documenting progress, and reporting changes to physicians. Your expertise will also involve educating patients on their plan of care, medication, therapy, diet, and exercise, as well as coaching family members on patient care, medication, and home safety.
Nurse Requirements:
- Active, unrestricted Registered Nurse (RN) license in Michigan
- At least one year of clinical experience as a Registered Nurse
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
