Job Description:
As a Home Health RN Case Manager, you will be responsible for providing high-quality, patient-centered care to individuals in their homes. Your duties will include:
- Conducting thorough assessments and developing individualized care plans
- Providing education and support to patients and their families on medication management, therapy, and healthy lifestyle habits
- Collaborating with interdisciplinary teams to coordinate patient care and ensure seamless transitions between settings
- Monitoring patient progress and adjusting care plans as needed

Required Skills and Qualifications:
- Active, unrestricted RN license in the State of Michigan
