Position Summary: The Family Case Manager at Interfaith Sanctuary plays a central role in guiding families toward stability and independence. This position is responsible for identifying guest needs, setting measurable goals, coordinating care with internal and external providers, and tracking progress across a variety of life domains, including housing, education, health, legal support, and employment. Case Managers serve as advocates, connectors, and mentors—helping guests navigate complex systems with compassion and consistency.
Primary Responsibilities:
Guest-Centered Case Planning
- Establish personalized case management plans with clear, measurable goals across the following areas:
- Mental and physical health
- Legal needs
- Housing instability
- Employment and education
- Family relationships
- General well-being
- Assist guests with securing vital documents: ID cards, birth certificates, and Social Security cards.
- Identify and address individual barriers to housing and stability.
Health & Medical Coordination
- Connect guests with appropriate healthcare resources:
- Medicaid/Medicare/waiver programs
- Primary care providers, specialists, and behavioral health partners (Terry Reilly, Full Circle, Trivium, Clarvida, Intermountain)
- Transportation (MTM, School Bussing)
- Coordinate care with crisis response units (e.g., mobile crisis, Clarvida).
- Support access to substance use disorder (SUD) treatment.
Housing Navigation
- Connect guests to housing programs including:
- VA, HUD-VASH, BCACHA, Section 8, Choice Voucher
- CATCH assessments and follow-up
- Assist with housing applications, waitlists, redeterminations, and appeals.
- Address barriers to housing through appropriate community referrals.
Legal & Financial Advocacy
- Support access to legal services for debt resolution, immigration, and government benefits.
- Help guests establish banking relationships, build credit, and develop budgeting skills.
Employment & Education Support
- Offer resume development and job-readiness guidance.
- Refer guests to partners for vocational training, GED programs, and employment opportunities (e.g., Idaho Youth Ranch, Easter Seals).
Social Security Navigation
- Guide guests through obtaining Social Security Cards and income verification letters.
- Assist with applications or challenges related to SSDI/SSI benefits.
Life Skills Navigation
- Guide guests in developing essential life skills such as budgeting, meal planning, cleaning, proper hygiene, and room sanitation.
- Connect guests with classes and support networks to further assist with these life skills.
Program Facilitation
- Help develop and refine comprehensive programming suitable for all age groups, encompassing subjects like budgeting, art therapy, meal planning, home organization, early childhood education, and communication and emotional regulation.
- Lead and facilitate various classes, including preschool and after-school programs for children, as well as life skills courses designed for parents.
Data Collection & Reporting
- Complete new intakes
- Collect data for a variety of reports
- Guest tracking through Mission Tracker:
- Effectively use online tracking system to look up guest stays, locker assignments and run a variety of reports
- Write case management and behavior tracking notes
- Upload and view guest documents
Community Collaboration
- Work in partnerships with housing, healthcare, and behavioral health providers.
Shelter Operations
- Assist with any day to day tasks that help the shelter run smoothly such as cleaning and organizing
- Assisting with meal preparation and distribution, sorting donations, and maintaining inventory of supplies.
- Supporting staff with administrative duties like filing, data entry, and answering phones, ensuring all operational aspects contribute to a safe and welcoming environment for guests.
- Ability to complete additional tasks as assigned with flexibility.
Qualifications:
- Bachelor’s degree in social work, psychology, public health, or a related field preferred; equivalent professional experience will be considered.
- Experience supporting vulnerable populations, particularly individuals experiencing mental health challenges or substance use disorders, is preferred.
- Comfortable using all Google Workspace applications (Docs, Sheets, Slides, Forms, Gmail, etc.) or Microsoft Office.
- Willingness and ability to learn HMIS, Mission Tracker, and apply case management best practices.
- Strong interpersonal, organizational, and advocacy abilities.
- Commitment to maintaining professional boundaries and demonstrating cultural humility.
Job Type: Full-time
Pay: $18.00 per hour
Expected hours: 40 per week
