The Shelter Case Manager provides case management services for families experiencing homelessness who are residing in IHN at Alpha House programs. This includes families at Alpha House East, Alpha House West, the Master Leasing program, and up to five families in the Community Mental Health Hoteling short-term stay program. In total, the Shelter Case Manager will work with up to 21 families across shelter and housing programs.
This role emphasizes a Housing First and trauma-informed approach, helping families secure and maintain stable housing while addressing barriers such as employment, childcare, health, and legal needs. The Shelter Case Manager collaborates with internal staff and external partners to coordinate holistic services that foster family stability.
General Responsibilities:
- Provide intake, assessment, and ongoing case management for up to 21 families across multiple shelter programs and up to 5 families in the CMH Hoteling partnership.
- Develop individualized service plans with families that are strengths-based, culturally responsive, and focused on stabilization goals (housing, income, employment, childcare, health, and family wellbeing).
- Conduct weekly case meetings with families per IHN program guidelines, adjusting frequency based on progressive engagement levels.
- Document all services, progress notes, and case plans accurately and timely using HMIS Service Point in compliance with confidentiality standards.
- Assist families in navigating housing applications, lease agreements, and transitions from shelter to permanent or transitional housing.
- Connect families to community resources including DHHS benefits, WIC, SSI/SSDI, childcare subsidies, transportation, medical/behavioral health services, and legal aid.
- Monitor families’ progress through home visits, in-shelter meetings, and phone/email check-ins, ensuring accountability and encouragement.
- Provide crisis intervention, conflict resolution, and referrals to address immediate needs (e.g., eviction prevention, food insecurity, safety concerns).
- Collaborate with Shelter Director, Operations Specialists, and external service providers to ensure coordinated, wrap-around support.
- Advocate for families within schools, courts, and social service systems as needed.
- Participate in weekly staff meetings, case reviews, and community provider meetings.
- Rotate responsibilities for supplemental programs such as the Food Gatherers distribution.
- Transport families to critical appointments when no other option is available.
- Perform other duties as assigned.
Minimum Qualifications:
- Bachelor’s degree in Social Work (BSW) or related human services field required; Master’s degree (MSW) preferred.
- At least 3 years of relevant experience working with families, housing, or homelessness services.
- Strong understanding of trauma-informed care, Housing First principles, and family systems work.
- Demonstrated success in working with diverse and vulnerable populations.
- Strong organizational, documentation, and communication skills.
- Ability to work independently and as part of a team.
- Proficiency with computers and database systems (HMIS preferred).
- Must have a valid driver’s license, reliable transportation, and meet insurability requirements.
