What You'll Do

In the Outreach SUD Counselor role, you’ll work closely with Interfaith’ s SUD Outreach team and will provide Substance Use Disorder (SUD) mobile outreach, screenings, and work closely with other homeless outreach teams and community partners to identify individuals experiencing homelessness with Opioid Use Disorder (OUD) and any co-occurring Substance Abuse

Disorder/Mental Health (SUD/MH) needs.

Essential Duties And Responsibilities

  • Conduct outreach and other engagement activities to identify eligible individuals and households, including households in shelter programs and affordable housing properties
  • Work as part of a multidisciplinary team to coordinate client care
  • Attend and participate in regularly scheduled outreach and care conferencing meetings
  • Collaborate with regional Emergency Department social workers, substance use disorder counselors, and mental health practitioners to facilitate a continuum of care and system navigation for clients
  • Conduct outreach and engagement from a harm reduction approach, developing rapport while identifying risks and needs
  • Conduct in-the-field screening and provide assessment and ongoing reassessments
  • Develop personalized strengths-based plans to reduce the negative physical and psychological impacts of drugs and alcohol
  • Help develop informal support systems as appropriate
  • Provide linkage to and support of engagement with recovery support services, such as vocational, educational, or crisis care services
  • Develop plans to reduce barriers to maintaining sobriety including; sober living, transportation, support groups, outpatient treatment and other needs as appropriate
  • Provide referral and coordination into Medication Assisted Treatment (MAT), detox and other treatment options
  • Collaborate across applicable agencies, providers and programs to enhance service and referral capacity
  • Complete accurate and thorough documentation in a timely manner
  • Maintain documentation of client meetings and interactions, including conversations and progress towards goals

You're passionate about making a lasting impact, whether supporting individuals directly or enhancing our organizational effectiveness. To thrive in this role, you'll:

  • High School Diploma or equivalent, required
  • Experience with individuals struggling with alcohol, drug, and mental illness or homelessness, required
  • Must be currently Registered by the California Consortium of Addiction Programs and Professionals (CCAPP), California Association of DUI Treatment Programs (CADTP) or the California Association of Drug/Alcohol Educators (CAADE) and consistently maintain certification once obtained, required
  • Experience working in an addiction treatment facility, social services or nonprofit setting, required.
  • Bilingual (English/Spanish), preferred

Skillsets / Additional Requirements

  • Ability to establish and maintain professional boundaries with clients
  • Experience with Motivational Interviewing techniques
  • Experience with Case Management
  • Experience with the ASAM Criteria Assessment Interview Guide
  • Sensitivity to cultural and socioeconomic characteristics of the population served
  • Must have reliable transportation and maintain a valid California driver’s license, be able to drive without restrictions, and have proof of personal vehicle insurance coverage and
  • insurability
  • Must successfully pass a criminal background and exclusion/disbarment check
  • Must successfully pass a pre-employment physical, tuberculosis test, and drug screen

Working Conditions / Physical Requirements

  • Must be able to perform work in an office setting
  • Must have the physical ability to sit for long periods of time, stand, walk, bend, kneel and uses hands, arms and legs for dexterity and balance on a frequent basis
  • Occasionally lifts, carries and balances objects weighing up to 30 pounds
  • May be subjected to adverse conditions, including contact from hostile people
  • Low to moderate exposure to contagious diseases (hepatitis, HIV, tuberculosis, etc.)
  • Low to moderate exposure of unpleasant and noxious fumes, odors, dust, temperatures, noise and inclement weather
  • Pulls and pushes such objects as file drawers, computers and similar office equipment and supplies
  • Prioritizes and multi-tasks work and projects requiring good memory, concentration and analytical thinking
  • Occasionally is required to perform the safe operation of a vehicle, office equipment and machines, and recognize and abate safety hazards within the workplace
  • Must be able to hear, see including distinction of colors, read and communicate verbally and in writing frequently with a wide range of people from divergent socio-economic backgrounds and origins
  • The noise level in the work environment is usually low

Location: Onsite/Escondido

Hours: Monday-Friday: 8am-4:30pm

Hourly Rate: $24.89-$28.55
