Position Overview
Under the direction of the Care Management Manager, the Health Coach serves as the primary nurse providing comprehensive health coaching and case management services across the full continuum of care — from health promotion and disease prevention to complex and end-of-life care.
This role combines strong clinical expertise with motivational interviewing and coaching skills to empower members to improve their health outcomes, navigate the healthcare system, and adopt sustainable lifestyle changes. The Health Coach partners closely with members, families, and providers to develop personalized care plans, remove barriers to care, and ensure high-quality, cost-effective service delivery.

Schedule & Training
- Training: Monday–Friday, 8:30 AM – 5:00 PM (approximately 1 month)
- Ongoing Schedule: 37.5 hours per week
- Monday–Thursday shifts may range between 8:00 AM – 8:30 PM
- Friday: 8:00 AM – 5:00 PM
- Late Shift Requirement: Must be available for late shift coverage three times per month (self-selected)
- Serve as the primary nurse for members across a wide spectrum of health needs, including wellness support, acute symptoms, chronic disease management, and complex conditions.
- Provide telephonic health coaching and case management services using clinical expertise and motivational interviewing techniques.
- Conduct comprehensive assessments and surveys to proactively identify member needs.
- Develop individualized care plans and establish measurable, mutually agreed-upon goals.
- Identify and address barriers to treatment adherence and lifestyle improvements.
- Facilitate ongoing telephonic follow-up to monitor progress and adjust care plans as needed.
- Support members calling with acute or chronic symptom concerns by providing appropriate clinical guidance.
- Collaborate with physicians and healthcare providers to coordinate care, particularly in complex medical situations.
- Educate members and families, serving as a liaison between the member, provider, and health plan.
- Connect members with telephonic, online, and community-based resources to support health goals.
- Promote preventive care and lifestyle improvement strategies during all interactions.
- Monitor quality of service, escalate safety concerns appropriately, and support quality-of-care tracking.
- Maintain accurate and compliant documentation in accordance with regulatory and accreditation standards.
- Participate in continuing education to maintain licensure and professional development.
- Current, active, and unrestricted Pennsylvania RN license (or Compact license including PA).
- Minimum of three (3) years combined experience in clinical nursing, case management, and/or disease management.
- Case Management Certification (CCM) preferred.
- Strong clinical knowledge and critical thinking skills.
- Exceptional communication, coaching, and problem-solving abilities.
- Experience with telephonic care management preferred.
- Ability to work flexible hours, including scheduled late shifts.
- Proficient in basic computer systems and documentation platforms.
- Demonstrated commitment to delivering exceptional member service.
