What You’ll Do
Member Care & Coaching
- Function as the primary nurse for members across the health continuum, from wellness and prevention to chronic and complex case management.
- Conduct outbound outreach to members with recent inpatient admissions, chronic conditions, or identified risk factors.
- Monitor inbound calls from members seeking coaching, clinical support, or benefit guidance.
- Utilize motivational interviewing and clinical expertise to establish goals, promote behavior change, and support self-management.
- Assess barriers to adherence and work collaboratively to help members follow evidence-based care plans.
- Perform telephonic assessments, screenings, and surveys to proactively identify needs.
- Develop individualized care plans and provide ongoing follow-up to measure progress and outcomes.
- Coordinate with physicians, providers, and interdisciplinary partners to streamline care.
- Provide education on conditions, treatments, medications, and resources.
- Assist members in navigating benefits, community resources, and support programs.
- Document all interactions in accordance with state, regulatory, and accreditation standards.
- Monitor services for quality of care, safety, and member satisfaction.
- Participate in audits, metrics tracking, and continuous quality improvement initiatives.
- Maintain licensure and complete all required ongoing training programs.
- Active, unrestricted Pennsylvania RN license (Compact accepted if it includes PA)
- 3 years of clinical experience in any combination of:
- Case Management
- Home Health
- Medical-Surgical
- Oncology
- Diabetes Management
- Case Management Certification (CCM) preferred
- Strong computer proficiency (Microsoft Office, multiple systems navigation)
- Exceptional communication skills with the ability to educate, coach, and motivate members
- Experience in hospital or insurance environments welcome
- Ability to work flexible hours and adapt to changing team needs
- Fully remote
