Position Overview
We are seeking a kind, compassionate, and motivated Nurse Practitioner to join our growing team. This role focuses on providing primary care services and palliative care consultations to geriatric patients across a variety of care settings. We are open to new graduates who are eager to learn and grow, as well as experienced providers.

Key Responsibilities
- Provide comprehensive primary care services to geriatric patients
- Conduct palliative care consultations with a focus on comfort, dignity, and quality of life
- Deliver care in assisted living, subacute rehab, long-term care, and home settings
- Collaborate with facility staff, families, and interdisciplinary care teams
- Develop and implement individualized care plans
- Maintain accurate and timely documentation
- Participate in ongoing learning and professional development
Qualifications
- Active Nurse Practitioner license in the state of New Jersey
- DEA license and CDS (Controlled Dangerous Substances) registration, or eligibility to obtain
- Ability to prescribe medications in accordance with state and federal regulations
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal skills
