Certified Nurse Practitioner – Family Health Center (Clinic Setting)

As we continue to grow, we are seeking a dedicated, experienced Certified Nurse Practitioner to join our Family Health Center team and help expand access to comprehensive, patient-centered care.
Position: Certified Nurse Practitioner (Full-time or Part-time)
Location: [Insert Clinic Location - e.g., Abington, Norristown, Pottstown or other VNACS Family Health Center site]
Reports to: Clinic Medical Director / Family Health Center Leadership
What You'll Do
- Provide comprehensive primary care services to patients of all ages (infants through seniors), including wellness exams, acute care, and chronic disease management.
- Conduct patient assessments, diagnose illnesses, order/interpret diagnostic tests, and develop and implement treatment plans.
- Prescribe medications and therapies, including managing ongoing medication regimens.
- Provide patient education, counseling, preventive health guidance, and support for health maintenance.
- Collaborate closely with other healthcare professionals - including RNs, support staff, and administrative team - to ensure coordinated, high-quality care.
- Maintain accurate patient records and documentation, including use of electronic health record (EHR) systems.
- Participate in ongoing professional development, training, and quality-improvement initiatives to support our mission of compassionate, patient-centered care.
Who You Are - Required Qualifications
- Master's or Doctoral degree in Nursing (or equivalent) from an accredited Nurse Practitioner program.
- Active Nurse Practitioner license in the state of Pennsylvania (or relevant state licensure) and national certification (e.g., family practice or adult-gerontology).
- Strong clinical judgment, diagnostic ability, and comfort managing a broad spectrum of health conditions and patient populations.
- Excellent communication skills, with a patient-centered and compassionate approach.
- Capacity to work independently and collaboratively as part of a multidisciplinary care team.
- Familiarity with electronic health records (EHR) and documentation best practices.
