Responsibilities
- Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations to develop tailored care plans.
- Manage discharge planning and coordinate with hospitals, clinics, and outpatient facilities to ensure smooth transitions of care.
- Provide expert case management for diverse patient populations including geriatrics, pediatrics, behavioral health, and those with infectious diseases or trauma histories.
- Perform diagnostic evaluations and monitor patient progress through regular follow-ups in the home or outpatient settings.
- Collaborate with multidisciplinary teams to optimize utilization management, inpatient/outpatient coordination, and post-acute care strategies.
- Maintain accurate medical records in compliance with HIPAA regulations and ensure documentation reflects current medical terminology and clinical findings.
- Educate patients and families on disease management, medication adherence, and health maintenance strategies to promote independence and recovery.
Qualifications
- Valid LVN or RN license with current state certification; additional experience as an LVN or RN highly preferred.
- Demonstrated critical care experience including ICU, emergency room, or Level I/II trauma center settings.
- Experience in hospital medicine, medical-surgical units, or post-acute care environments such as skilled nursing or rehab facilities.
- Strong knowledge of physiology, diagnostic evaluation techniques, discharge planning processes, and case management principles.
- Familiarity with hospital records systems, medical terminology, HIPAA compliance, and utilization management protocols.
- Background working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, or those requiring infectious disease care.
