Continuity of Care Navigator - QMHP

Job Summary: Continuity of Care Navigators perform moderately complex coordination work, provide preliminary assessment of a client's presenting psychiatric and/or co-occurring disorder(s), develop a plan to provide linkage to services sought or needed by the client, and to coordinate appropriate services to facilitate each client's success in achieving and maintaining the maximum possible level of independent functioning. This position is community-based and provides short term services to both with both adolescent and adult clients admitted to/discharged from the local network private psychiatric hospitals in Travis County. The services provided will focus on linkage to resources and aftercare, decreasing re hospitalization and ensuring that contractual obligations pertaining to continuity of care are being met.

This position serves individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. The services that Continuity of Care Navigators provide require frequent coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Continuity of Care Navigators provide coordination and discharge planning to assure that the individual is discharged to the least restrictive environment. Services may include coordination to assist the client in obtaining or maintaining employment, housing, medical care, dental care, food, clothing, utilities, financial assistance, transportation, legal assistance, or any other basic human need. Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safer care, and foster a supportive and welcoming environment. QMHPs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD.

Staff members in this position are working under the supervision of a clinically licensed professional who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. At this level, clinical assignments may be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision.

Services provided may include individual, family and/or group skills training using evidence-based interventions, crisis assessment, and case management. Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.

Regular, predictable attendance is considered to be an essential requirement for this position.

Education:

Required:

• Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:

• Current and valid driver's license and a clean driving record.

Experience:

Preferred:

• Bilingual in Spanish.

Required:

• Minimum of 2 years of crisis experience.

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00 AM - 5:00 PM 

Salary: $24.18 - $25.39 an hour, based on education.
