JOB DESCRIPTION

Summary
The Intensive Case Manager (ICM) provides appropriate intensive case management services to refugees or other eligible immigrant populations who have resided in the US for less than 5 years. The ICM program fosters the successful and integration of especially vulnerable refugees, asylees, Special Immigrant Visa (SIV) holders, Afghan/Ukrainian/Cuban/Haitian Humanitarian Parolees. The ICM program aims to effectively support vulnerable individuals that have difficulty navigating systems and attaining self-sufficiency in their new or existing community. The ICM case manager will provide intensive support to large families, women-at-risk, elderly refugees, refugees that identify as LGBT, refugees with significant physical or mental disabilities, and other vulnerable populations as needed.

Essential Functions

  • Provides intensive case management to address needs that extend beyond the initial resettlement period, specifically focusing on medical, educational, housing, and clients’ special needs.
  • Develops individualized, holistic service plans based on client needs.
  • Conducts RHS-15 mental health screening for all ICM clients and makes timely, appropriate referrals to mental health care providers.
  • Networks and develops personal contacts with other social service providers, including both mainstream providers and those that serve refugees exclusively.
  • Uses contacts and networking to create and maintain a database of client services to supplement those that Inspiritus provides.
  • Develops and facilitates periodic community support groups for Inspiritus clients to promote community connections and teach healthy coping strategies during the integration experience.
  • Arranges client appointments and provides schedules to clients and other individuals who may be assisting clients (e.g., Inspiritus case managers, interpreters, volunteers, etc.).
  • Provides ongoing orientation about health-related issues, including Medicaid education, identifying and selecting physicians who provide language appropriate services, and basic public health counseling.
  • In coordination with Community Engagement Specialist, refer clients to one-on-one volunteer mentoring/First Friend program.
  • Assists ICM clients in resolving emergency issues.
  • Keeps Client Services Manager apprised of difficult cases.
  • Serves as interpreter when language skills are applicable for case services.
  • Maintains personal and professional boundaries in the performance of the job.
  • Forges mutually respectful partnerships with clients, co-workers, and employers.
  • Participates in Inspiritus functions and public relations activities.
  • Participates in in-service training and conferences related to the programs.
  • Conforms with and abides by all regulations, policies, work procedures and instructions.
  • Complies with the various federal, State, or local laws that apply to and regulate the job responsibilities.
  • Participates in training, fundraising events and other Inspiritus activities and initiates such as performance and quality improvement (PQI).

Work Environment

While performing the responsibilities of this job, the employee travels by automobile and is exposed to changing weather conditions. Member of mobile workforce, and 50% of time spent traveling by automobile or in the field. Makes home visits to refugee families and transports clients to appointments as needed. Is expected to work outside regular working hours if necessary to accommodate clients’ needs. May perform job functions at out-posted location as assigned.

Physical Demands

Regularly required to talk and hear. Required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls: and reach with hands and arms. Required to operate personal vehicle. Employee frequently lifts and or moves up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

Position Type and Expected Hours of Work

This is a full-time position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m.  

Travel:  Up to 50% locally
 

Required Education and Experience

  • Bachelor’s degree with a concentration in a relevant field (social work, counseling, public health, or psychology)
  • Two years of previous direct work experience in a culturally diverse environment or an equivalent combination of education and experience.
  • Ability to organize multiple client-related activities.
  • Excellent oral and written communication skills in English.
  • Ability to balance the needs of the client and the employer.
  • Conviction about the capacity of people to grow and change.
  • Ability to set limits and maintain the helping role of the practitioner.
  • Excellent computer skills, accurate typing, and knowledge of Microsoft Word, Excel, and Outlook.
  • Must have the ability to communicate with all persons in a culturally competent manner.
  • Must be sensitive to the service population’s cultural and socioeconomic characteristics.
  • Must own a personal multi-passenger vehicle, hold a valid Georgia/Tennessee/Alabama Driver’s license, and have personal auto insurance and a good driving record.
  • Or an equivalent combination of education, experience, competencies, skills, and knowledge that ensures the successful completing of the essential job responsibilities.

Preferred Education and Experience

  • Master’s degree with a concentration in a relevant field (social work, counseling, public health, or psychology) and 2 or more years of applicable human services experience
  • Experience in case management, health advocacy, community outreach, and partnership development preferred.
  • Experience in maternal and child health, reproductive health, medical and insurance systems, or health education preferred
  • Knowledge of immigration and/or refugee issues
  • Reads, writes, and speaks a relevant foreign language
