Qualifications:
- RN license (IL)
- Auto insurance along with Valid Driver's license
- Ability to travel to multiple locations.
- minimum 1 year home health experience
- Previous experience with EMR system: Home Care Home Base
Responsibilities:
- Make an initial health evaluation and individualized plan of care.
- Administer medications and assist with pain management.
- Clean and dress wounds
- Document symptoms and vital signs
- Monitor patient health and update care plan accordingly
- Conducting comprehensive assessments of patient's health conditions and developing personalized care plans in collaboration with physician
- Administering medications, including intravenous infusions, and managing wound care
- Write reports and communicate with doctors after visits.
