Job Title: Transitional Living Program Case Manager

Schedule: Monday- Friday 9 am-5 pm (Occasional evenings and weekends required)
Job Summary
The Transitional Living Program (TLP) Case Manager provides direct services and support to youth and young adults experiencing homelessness who are transitioning to independent living. The Case Manager works closely with program participants to develop individualized service plans, connect them with community resources, and support their progress toward stable housing, education, employment, and life skills development.
This role utilizes a trauma-informed and strengths-based approach to help participants build independence and achieve long-term stability.
Key Responsibilities
Conduct intake assessments and develop individualized service plans for program participants.
Provide ongoing case management support, including goal setting, life skills coaching, and progress monitoring.
Maintain regular one-on-one meetings with participants to assess needs and support their transition to independent living.
Assist participants with securing permanent housing, employment, education, and healthcare services.
Support participants in developing budgeting, time management, and other independent living skills.
Maintain accurate and timely case documentation, including case notes, service plans, and progress reports.
Enter and manage participant data in the Homeless Management Information System (HMIS) or other required databases.
Ensure all documentation complies with agency policies and grant funding requirements.
Assist with program reporting and outcome tracking as required by funders.
Provide crisis intervention and de-escalation when participants experience emergencies or emotional distress.
Monitor participant safety and ensure compliance with program guidelines and house rules.
Collaborate with the Program Supervisor to address behavioral or safety concerns.
Participate in a rotating on-call schedule if required.
Coordinate with community partners, including housing providers, employment programs, healthcare providers, and educational institutions.
Refer participants to supportive services such as mental health counseling, substance use treatment, and legal assistance.
Participate in case conferences and multidisciplinary team meetings.
Advocate on behalf of participants to help them access community resources and opportunities.
Assist with program activities, workshops, and life skills training sessions.
Support participants in maintaining a safe and respectful residential environment.
Participate in program meetings, trainings, and professional development opportunities.
Required Qualifications
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field preferred.
Equivalent experience in social services may be considered.
1–2 years of experience working with youth, young adults, or individuals experiencing homelessness.
Experience in case management, housing services, or residential programs preferred.
Understanding of Trauma-Informed Care, Positive Youth Development, and Harm Reduction principles.
Strong communication, organizational, and problem-solving skills.
Ability to work effectively with diverse populations.
Proficiency in Microsoft Office or Google Workspace.
Requirements
Valid driver’s license and reliable transportation (preferred).
Ability to pass a background check.
CPR/First Aid certification or willingness to obtain.
Work Environment
Work is performed in an office and residential program setting.
May require occasional evening or weekend hours.
Local travel may be required for participant meetings, housing visits, or community appointments.
Ability to navigate residential facilities and occasionally lift up to 20 lbs.
