The Transportation Coordinator is responsible for coordinating and dispatching transportation routes. This includes both in-house and outside appointments. Transportation Coordinator’s will also be responsible for participating in Inter Disciplinary Team (IDT) and Care Planning meetings.
Essential Job Functions
Duties include, but not limited to:
- Participate as a member of the Inter Disciplinary Team and Care Planning meetings, and report changes in participants’ baseline status to appropriate staff.
- Collaborate with applicable departments to coordinate transportation for all participants scheduled clinic and outside provider appointments, day center and outings.
- Involved in the implementation of Quality Improvement activities.
- Develop and maintain transportation routes manually and with transportation software.
- Dispatch and communicate new routes to drivers with their participant routes.
- Prepare transportation metrics and driver performance and safety reporting for Management, as needed.
- Prepare monthly transportation ride count reporting.
- Work with Transportation Assistant(s) and Drivers to ensure vehicle maintenance and repairs.
- Work with other departments to coordinate additional routes, meal delivery, DME and/or care packages.
- Attend and participate in staff meetings, in-services, projects, and committees as assigned.
- May be asked to drive a company vehicle and complete routes to provide support for the department.
- Understands and navigates the Electronic Health Records system.
- Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.
- Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
- Be flexible in the schedule of hours worked.
- May require use of personal vehicle.
Knowledge, Skills, and Abilities
- Knowledge of the area in which Innovative Integrated Health is located.
- Ability to cope with mental and emotional stress related to the position.
- Function independently, have flexibility, and personal integrity.
- Must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads.
- Effective written and oral communication skills.

- Ability to access all areas of the center throughout the workday is required.
- Ability to lift up to 50 pounds occasionally, up to 25 pounds frequently, and 15 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
- Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
- Ability to communicate verbally and in writing with an excellent comprehension of the English language is required.
- Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
- Minimum of one (1) year of documented experience in working with a frail or elderly population.
- Working in other medical transport preferred.
- High school graduate or equivalent preferred.
- Must have a valid California Driver’s License.
- Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
