About the Role
We’re seeking a compassionate and experienced Visiting After-Hours Registered Nurse (RN) to provide high-quality care to participants in a home-based setting. This is a work-from-home position with field visits as needed to participants’ residences. Will be home care visits during the day Saturdays and Sundays. The RN will triage after-hours calls, provide clinical guidance, and respond in person when medically necessary.
This role is ideal for a nurse who thrives on autonomy, critical thinking, and delivering care that truly makes a difference.
Key Responsibilities
- Respond promptly to after-hours calls from participants, families, and staff.
- Perform phone triage and determine appropriate next steps, including home visits when required.
- Conduct thorough assessments, follow care plans, and collaborate with the clinical team for coordinated care.
- Provide education and emotional support to participants and their families, including end-of-life counseling.
- Document all care and communication accurately in the electronic medical record.
- Maintain a safe, ethical, and compassionate approach to all patient interactions.
- Participate in handoffs, team huddles, and ongoing education as needed.
Schedule & Contract Details
- 3-Month Contract
- After-Hours Schedule: Friday 4:30 PM – Monday 8:30 AM
- Some Holidays required
Qualifications
Required:
- Current RN license (State of Colorado)
- Current First Aid and BLS (American Heart Association or American Red Cross)
- Minimum of 2 years of healthcare experience, including at least 1 year in home health or geriatrics
- Reliable personal transportation, valid driver’s license, and proof of auto insurance
- Associate’s Degree in Nursing
Preferred:
- Bachelor’s Degree in Nursing (BSN)
- 3 years of experience in geriatrics, hospice, or end-of-life care
- Bilingual (Spanish, Russian, Punjabi, or Hmong)
