Registered Nurse (RN) Case Manager
Location: Brunswick, GA | Job Type: Direct Hire or Contract to hire

The Role (Key Responsibilities)
- Provides services in accordance with the plan of care.
- · Makes the initial evaluation visit and regularly reevaluates the patient’s needs.·
- Initiates the plan of care and necessary revisions.
- Prepares clinical and progress notes for each patient visit in a timely manner as per agency policy.
- Experience: 2 years of experience in [Field/Industry].
- Education: RN License, BSN preferred.
- Skills:· Ability to exercise initiative and independent judgment.
- Soft Skills: Strong communication, problem-solving, and train other associates
- Certifications: BLS
