Home Health RN / Case Manager – Athens, GA
Athens, GA | $85,000/year Mileage Reimbursement
Full-Time | Primarily 8:00 AM – 5:00 PM

We're seeking an experienced Home Health RN / Case Manager to provide high-quality, patient-centered care throughout the Athens, GA service area and surrounding counties. In this role, you'll manage your own caseload, coordinate care with physicians and interdisciplinary teams, and help patients remain safely and comfortably in their homes.
Territory / Service Area
This position will primarily cover Athens-Clarke County and the surrounding service area, including:
- Clarke County
- Barrow County
- Jackson County
- Madison County
- Oconee County
- Oglethorpe County
- Walton County
Candidates should be comfortable traveling throughout the assigned territory to complete patient visits.
What You'll Do
- Complete initial patient assessments and develop individualized plans of care.
- Manage and coordinate ongoing patient care and case management.
- Perform routine home visits and reassess patient needs.
- Educate patients and families on disease management and treatment plans.
- Communicate changes in patient condition with physicians and the care team.
- Complete accurate clinical documentation in a timely manner.
- Process physician orders, certifications, recertifications, and discharge summaries.
- Supervise and mentor nursing staff as appropriate.
- Participate in quality improvement initiatives and ensure compliance with agency standards.
Qualifications
- Active Registered Nurse (RN) license — Compact or Georgia State.
- Minimum of 2 years of nursing experience; Home Health experience strongly preferred.
- Strong assessment, communication, and clinical decision-making skills.
- Comfortable working independently and managing a patient caseload.
- Valid driver's license and reliable transportation.
- Ability and willingness to travel throughout the assigned service area.
