Overview
We are seeking a compassionate and experienced Case Manager / Group Facilitator to join our mental health and behavioral health services team. This role involves providing comprehensive case management, facilitating group psychoeducation and experiential sessions, and supporting individuals with diverse mental health needs. The ideal candidate will have a strong background in clinical counseling, crisis intervention, and working with various populations. Responsibilities
- Conduct thorough patient assessments and diagnostic evaluations utilizing medical terminology and ICD coding (ICD-10).
- Develop individualized case management treatment plans incorporating client needs including, but not limited to, education, vocation, legal coordination, medical coordination, and discharge planning.
- Facilitate group therapy sessions focusing on issues such as grief, trauma (PTSD), addiction, chronic pain, gender issues, and behavioral management.
- Provide crisis management and crisis intervention to ensure client safety during escalated episodes.
- Coordinate care across multidisciplinary teams while maintaining HIPAA-compliant medical documentation and accurate medical records.
- Utilize telehealth platforms for remote case management sessions when appropriate.
- Support patients through intake processes and ongoing case management activities.
- Engage in research activities related to behavioral health to improve treatment outcomes.
Requirements
- Strong background working with substance use disorder, its population, and families.
- Experience with crisis intervention techniques and crisis management strategies.
- Knowledge of medical terminology and familiarity with ICD coding (ICD-10).
- Experience in outpatient or inpatient settings within mental health or behavioral health clinics.
- Ability to handle sensitive situations involving PTSD care, chronic pain counseling, gender issues counseling, etc.
- Proficiency in medical documentation standards ensuring compliance with HIPAA regulations.
- A bachelor's degree in psychology or equivalent, Certified Addiction Counselor (CIT or CAC1 ). This position is integral to providing high-quality mental health services that promote recovery and resilience across diverse populations while fostering a compassionate therapeutic environment for all patients served.

Pay: $42,500.00 - $47,500.00 per year
