Job Summary
Position Type: On-Demand / Per Diem (Independent Contractor or PRN Employee) Compensation: $70-$150 per 50-minute remote session (billed per session completed)
Sessions Offered: 1-2 Weekly 50-minute primarily remote sessions (flexible scheduling based on participant and program needs)
Inland Circle THP-NMD, a Transitional housing provider for NMDs ages 18-24 in Moreno Valley, Ca. We are seeking a dedicated, and compassionate Social Work Personnel to join our dynamic team. In this per diem role will provide comprehensive social support services to our THP-NMD program participants, ensuring they receive personalized care that addresses their emotional, psychological, and social needs. Your expertise will help facilitate positive health outcomes through effective case management, counseling, and community resource coordination. Requirements
- Master's Degree from an accredited graduate school, in one of the following areas- social work or social welfare, marriage, family and child counseling, child psychology, child development.
- At least three semester units or 100 days of field practice or experience in a public or private social service agency setting at the Master's Degree level.
- At least nine semester units of coursework related to children and families or 18 months of experience working with children and families.
- At least three semester units in working with minority populations; or six months of experience in working with minority populations; or six months in-service training in working with minority populations within the first year of employment as a condition of employment.
- At least three semester units in child welfare, or two years of experience in a public or private child welfare social services setting.
Duties
- Conduct thorough patient assessments to identify social, emotional, and behavioral needs using clinical counseling and diagnostic evaluation techniques.
- Develop individualized care plans incorporating behavioral therapy, applied behavior analysis, and mental health counseling strategies tailored to each patient’s circumstances.
- Provide both individual and group therapy sessions focused on behavioral health, aging issues counseling, geriatrics, and other specialized areas such as Christian counseling or psychodynamic therapy.
- Coordinate with medical teams to ensure accurate medical documentation and maintain detailed medical records in compliance with HIPAA regulations.
- Facilitate intake processes by gathering comprehensive patient information, conducting case management activities, and connecting patients with community resources for home & community care.
- Support patients through sensitive patient care practices while respecting privacy standards; assist with behavioral health interventions and mental health support.
- Engage in ongoing professional development related to medical terminology, diagnostic evaluation methods, and current best practices in social work and behavioral health fields.
Experience
- Proven experience in social work or behavioral health settings, ideally with home & community care experience or in clinical counseling environments.
- Strong background in behavioral therapy, applied behavior analysis, and mental health counseling techniques.
- Familiarity with medical terminology, medical documentation procedures, and maintaining HIPAA-compliant medical records.
- Experience conducting patient assessments, intake interviews, and diagnostic evaluations within diverse populations including geriatrics or aging issues counseling.
- Demonstrated ability to provide individual and group therapy sessions effectively across various therapeutic modalities such as psychodynamic therapy or Christian counseling.
- Knowledge of case management principles combined with excellent communication skills for collaborating with multidisciplinary teams.
