SUMMARY

Oversees the delivery of care and services and the care resources to the patients based on the assessed conditions and the
personalized plan of care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Works with the admitted patients, assessing their clinical conditions and needs with the IDT in order to develop
personalized plan of care.
  • Monitors, coordinates and communicates the delivery of care and services to the patients, family members,
physicians, IDT and Third Party Case Manager.
  • Provides Resident Assessment and Planning Tools. May include early identification of potential problems that might
adversely impact plan of care.
  • Coordinates the care resources such as the ancillary and routine services throughout the skilled care days of the
patients.
  • Facilitates patient and family education and arrange community resources as needed in preparation for a safe and
successful discharge to the community.
  • Provides periodic assessment and summaries in coordination with Interdisciplinary team to insure for continued
authorization of care. May assist with new/readmission assessment (for RN) or data collection (for LVN) when
assigned by the Director of Nursing.
  • May provide nursing care.
  • Confirms appropriate documentation to support level of care and medical necessity.
  • Confirms appropriate level of care to include utilization limits and outliers/exclusions.
  • Conducts clinical cost and benefit analysis for every referral.
  • Acts as a liaison to the business office to facilitate collection of claims.
  • Completes required forms and documents in accordance with company policy and federal and state regulations.

ENTRY QUALIFICATIONS

  • Graduate of an accredited School of Nursing
  • Valid State licensure as an LPN, LVN, or RN with SNF working experience.

SUPERVISORY RESPONSIBILITIES

  • Coordinates care being provided by the Licensed Nurses and the Therapists under assigned case load.

PHYSICAL DEMANDS AND ENVIRONMENT

Shift work may be required. For travelling Case Managers (who are handling more than 1 bldg.) may be able to travel from
one facility to another. Employee is occasionally exposed to blood or other body fluids, fumes, or airborne particles and toxic
or caustic chemicals. In compliance with applicable law, reasonable accommodations may be made to enable individuals with
disabilities to perform the essential functions of the position. Ability to function as a team leader, and be flexible and adaptable
to change. Must be self-motivated and have analytical skills, and operate various job related equipment (computer & phone).

OTHER REQUIREMENTS

Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident’s
protected health information. Monitors and oversees facility’s efforts to comply with company policies, procedures and state

relative to the confidentiality of resident’s protected health information.
