Inglemoor Rehabilitation and Care Center, a 5 Star Post Acute/Long term Care facility, located in Essex County, NJ seeking a dedicated, experienced MDS Registered Nurse to join our heath care team on a PT basis. MDS RN will have at least 3 years’ experience in MDS and long term care regulations. Competitive salary and benefits
Responsibilities:
- Coordinate and maintain the Minimum Data Set (MDS process for all long term residents, ensuring accurate and timely assessments).
- Develop and implement care plans that meet a resident centered plan of care, in collaboration with the interdisciplinary team.
- Conduct MDS assessments, evaluations, and progress notes, ensuring compliance with regulatory requirements
- Provide education and training to staff on MDS process and documentation requirements
- Utilize EMR and HER systems effectively for documentation and reporting purposes
- Familiar with EPIC, PCC and EMR/EHR, CMS 5 star
- Strong analytical skills with attention to detail for documentation review and coding accuracy
- Ability to work independently as well as part of IDCP team
- Excellent communication skills
