Role Description
This is a full-time on-site role for our client, a not for profit licensed skilled nursing fscility, based in Morristown, NJ, for an MDS Coordinator. The role involves coordinating and managing the assessment process for residents, facilitating the development of comprehensive care plans, and ensuring compliance with all applicable regulations. The MDS Coordinator will work closely with the care team to assess and address the needs of residents, provide quality assurance, and optimize reimbursement processes.
Qualifications
- Must possess a current and valid Registered Nurse (RN) license.
- Experienced in Care Planning and creating effective Care Plans tailored to resident needs.
- Proficiency with MDS (Minimum Data Set) assessments and federal/state compliance requirements.
- Background in Geriatric Nursing with a focus on ensuring quality care for older adults.
- Strong organizational, communication, and documentation skills required.
- Attention to detail and ability to work collaboratively with interdisciplinary care teams.
- Knowledge of Medicare and Medicaid regulations is highly desirable.
- Minimum of 2 years of experience in long-term care or a related field is preferred.
