Required Skills and Qualifications:
- Has an active MA RN or LPN license
- Geriatric or SNF experience preferred
- Strong communication skills and ability to work independently and as part of a team
- Organized, professional, and committed to high ethical standards
Duties and Responsibilities:
- Coordinate physician visits, rounds, and resident updates
- Manage admissions, hospital returns, and care plan updates
- Monitor resident conditions, nutrition, and report changes
- Supervise nursing staff and assist with ADLs
- Coordinate care with other departments and complete required documentation
- Other duties as assigned.
Job Type: Per Diem, Temporary
Schedule: 8-hour shift/ 12-hour shift/16-hour shift, All shifts available, Weekends
License/Certification: BLS/CPR Certification, RN or LPN License
