Case Manager

About the Role:

Infinity Love Home Care Agency is seeking a highly organized, mission-aligned Case Manager to oversee patient care coordination and caregiver management. The ideal candidate is a proactive professional with a deep understanding of the home care landscape and a commitment to maintaining the highest standards of clinical and administrative compliance.

Preferred Qualifications:

• Experience: Minimum of 2 years of direct experience in Home Care Case Management.
• Technical Skills: Proficiency in HHA Exchange is strictly required.
• Knowledge: Deep understanding of home care regulations, Medicaid waivers, and EVV (Electronic Visit Verification) requirements.
• Credentialing: Must be fully credentialed and able to pass a rigorous background check and child abuse clearance.
• Attributes: Strong communication skills, the ability to multi-task in a fast-paced environment, and a "patient-first" mentality.

Responsibilities:

• Caseload Management: Oversee a dedicated caseload of patients, ensuring all care plans are followed and updated in accordance with state regulations.
• HHA Exchange Oversight: Manage daily scheduling, authorization tracking, and compliance documentation within the HHA Exchange platform.
• Caregiver Coordination: Act as the primary point of contact for Home Health Aides (HHAs); manage caregiver schedules, performance, and credentialing updates.
• Compliance & Quality Assurance: Ensure all patient files and caregiver credentials are fully "audit-ready" and compliant with PA Department of Health standards.
• Collaboration: Work closely with the clinical director and intake department to transition new clients smoothly into service.
