Position Summary
The IHS/Homemaking Direct Support Professional provides person-centered services to individuals with disabilities in their home and community. This position supports individuals in maintaining independence, developing daily living skills, and ensuring a safe, clean, and healthy living environment in accordance with Minnesota 245D regulations.
Essential Duties & Responsibilities
Individualized Home Supports (IHS)
- Provide direct support services based on the individual’s Coordinated Service and Support Plan (CSSP) and addendum.
- Assist with skill development in areas such as:
- Community integration
- Social skills
- Budgeting and money management
- Meal planning and preparation
- Medication reminders (if applicable)
- Support individuals in achieving personal goals.
- Promote independence, dignity, and choice.
- Provide behavioral support strategies as outlined in support plans.
- Ensure health and safety standards are maintained.
Homemaking Services
Assist with general household tasks including:
- Cleaning and organizing
- Laundry
- Dishwashing
- Changing linens
- Trash removal
- Assist with meal preparation and grocery shopping.
- Help maintain a sanitary and safe home environment.
- Provide light assistance with errands and essential household management tasks.
Documentation & Compliance
- Complete accurate and timely service documentation.
- Follow all 245D policies and procedures.
- Maintain confidentiality in accordance with HIPAA regulations.
- Report incidents, emergencies, and health/safety concerns per agency policy.
- Participate in required staff meetings and trainings.
Qualifications
- High school diploma or GED required.
- Experience working with individuals with disabilities preferred.
- Ability to pass background study per DHS requirements.
- Valid driver’s license and reliable transportation/ valid car insurance (if transporting clients).
- Strong communication and organizational skills.
- Ability to work independently and maintain professional boundaries.
- Speaks English and Russian
Physical Requirements
- Ability to lift up to 25–50 pounds (if required).
- Ability to stand, bend, kneel, and perform light physical tasks related to cleaning and support.
Work Environment
Services are provided in the individual’s home and community settings. Schedule may include evenings, weekends, or flexible hours depending on client needs.
