- Plan, develop, and oversee the Social Services Department to ensure residents’ psychosocial, emotional, and environmental needs are met
- Conduct comprehensive psychosocial assessments upon admission and at regular intervals to identify individual needs, preferences, and goals
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
- Lead and coordinate discharge planning, ensuring safe and appropriate transitions to home, hospitals, assisted living, or other care settings
- Advocate for residents’ rights, dignity, and independence, ensuring compliance with all federal, state, and local regulations (e.g., CMS guidelines)
- Provide counseling, crisis intervention, and emotional support to residents and their families related to illness, adjustment, and long-term care placement
- Serve as a liaison between residents, families, staff, and external agencies to promote clear communication and continuity of care
- Facilitate and participate in care plan meetings, family conferences, and resident council meetings
- Maintain accurate, timely, and compliant documentation in accordance with regulatory standards and facility policies
- Supervise, mentor, and evaluate social services staff, including hiring and ongoing training as needed
- Address concerns related to abuse, neglect, or grievances, ensuring proper reporting and resolution in line with regulations
- Connect residents and families with community resources, financial assistance programs, and support services
- Support quality improvement initiatives and surveys by ensuring the department meets all compliance and performance standards
- Bilingual preferred
Salary : $80,000 - $90,000
