Job Title: Integrated Case Manager

Location: Detroit

Duration: 13-week contract (possible extension)

Shift: 8:00 AM - 4:30 PM (weekend rotation)

Pay Rate: $57/hour


Duties:

  • Conduct patient assessments (clinical, social, and functional needs)
  • Develop and manage individualized care plans
  • Coordinate care and ensure smooth transitions across settings
  • Collaborate with multidisciplinary teams for complex cases
  • Educate and support patients and families
  • Monitor patient progress and adjust care plans as needed


Requirements:

  • 5 years of case management experience with 2 years recent acute inpatient experience
  • Current MI RN licensure or LMSW
  • Discharge planning experience
