Compass Park, a premier 320-acre continuing care retirement community in the heart of Johnson County, just outside Indianapolis, is seeking a highly experienced and compassionate Registered Nurse to join our growing Home Health Leadership Team.

This is a key leadership opportunity for a nursing professional who is passionate about high-quality, person-centered care and has a strong background in home health clinical operations, regulatory compliance, and staff development.

Position Overview:

The Home Health Clinical Services Director is responsible for the strategic planning, coordination, and oversight of all direct and indirect services provided by the Home Health Agency. This leader ensures clinical excellence, regulatory compliance, and optimal patient outcomes, while overseeing agency performance and staff development.

Key Responsibilities:

• Direct and oversee clinical operations, including patient admissions, care coordination, and discharge planning.
• Assess and verify eligibility criteria for home health services in accordance with Medicare, CHAP, and Joint Commission standards.
• Collaborate with senior leadership on strategic planning, budgeting, and continuous improvement initiatives.
• Lead the development and implementation of policies, procedures, and care protocols to ensure delivery of high-quality, evidence-based care.
• Serve as a liaison between the agency, patients, families, and physicians; effectively communicate the scope of services and available resources.
• Maintain compliance with state and federal regulations and participate in internal audits, quality assurance reviews, and regulatory surveys.
• Develop and implement orientation, in-service training, and continuing education programs to foster professional growth and staff retention.
• Provide clinical supervision and performance evaluations for all care delivery staff, supporting career development and accountability.
• Monitor data for Quality Assurance and Performance Improvement (QAPI) programs and use findings to enhance patient care and operational efficiency.
• Act as Agency Administrator in their absence, representing leadership with professionalism and integrity.

Qualifications:

• Registered Nurse (RN) licensed in the state of Indiana
• Graduate of an accredited nursing program; BSN preferred
• Minimum 3 years of progressive leadership experience in home health or healthcare management
• Strong knowledge of Medicare regulations, CHAP, Joint Commission, and home health operations
• Demonstrated leadership and staff management skills in a clinical setting
• CPR certification required
• Excellent communication, organizational, and interpersonal skills
• Valid driver’s license and insured personal vehicle as required for travel
