Aging Life Care Manager (RN, PT, OT, MSW) – Independent You, Senior Services

Independent You, Senior Services, is seeking a compassionate, motivated professional with a clinical background (RN, PT, OT, or MSW) to join our dedicated team as an Aging Life Care Manager.

Since 2010, our mission has been to provide seniors and their families with trusted guidance, support, and solutions that enhance quality of life and peace of mind. As a member of our care management team, you’ll act as a partner and advocate for clients and their families, helping them navigate the challenges of aging with dignity, safety, and confidence.

Key Responsibilities

·   Conduct comprehensive assessments of clients’ physical, mental, emotional, and social well-being

·   Collaborate with clients and families to identify both immediate needs and long-term care goals

·   Develop and implement individualized care plans tailored to client strengths, needs, and preferences

·   Build and maintain relationships with families, healthcare providers, legal/financial representatives, and community partners

·   Advocate for clients’ rights and intervene when safety, neglect, or abuse concerns arise

·   Provide crisis intervention, supportive counseling, and guidance for families navigating complex care decisions

·   Document assessments, care plans, and progress notes in the electronic client record

·   Regularly evaluate and adjust care plans to ensure effective, client-centered outcomes

Qualifications

·   Professional degree required

·   Minimum of 2 years’ experience working with aging adults in a supervised setting

·   Strong organizational, communication, and problem-solving skills

·   Proficiency with basic computer programs and documentation systems

·   Reliable transportation and a clean driving record
