Independent Living Partners is seeking experienced and driven IHS workers to join our growing team. Individualized Home Supports (IHS) is an active role that involves bending, walking, and hands-on work. You will not only assist clients with various activities but also participate alongside them to ensure tasks are completed efficiently. In many cases, you will independently be completing activities and tasks that help individuals maintain a dignified and independent life.
Primary Responsibilities
- Complete laundry and general housekeeping tasks: cleaning, mopping, vacuuming etc
- Assist individuals in organizing their lives and working toward personal goals
- Schedule meetings and appointments
- Arrange medical and social services
- Support individuals with task completion and problem-solving
- Assist with meal planning, cooking, and meal preparation
- Arrange or provide transportation
- Support individuals in safely accessing the community for socialization with friends, family, and peers
- Support individuals in safely accessing the community for recreational activities
- Help develop communication and social skills
- Assist with self-care routines
- Help with paperwork and personal organization
- Assist individuals with grocery shopping and other essential errands
- Create a work schedule that aligns with each client's needs
- Complete daily case notes and time sheets
- Provide assistance with Activities of Daily Living (ADLs) to maintain client dignity
Training and Qualifications
- Meet DHS qualifications for Individualized Home Supports with training
- Experience working with individuals with disabilities
- Patience and understanding when working with individuals experiencing mental health and behavioral challenges
- Associate's degree preferred
- First Aid certification required
General Skills and Requirements
- Ability to work with individuals with disabilities and chronic illnesses
- Experience supporting individuals with mental health conditions
- Ability to identify and teach skills that promote client independence
- Completion of 245D HCBS training upon hire and annually thereafter
- Ability to pass a Minnesota Department of Human Services background study
- Valid driver's license with fewer than three moving violations in the past three years, including DUIs
- Reliable vehicle with valid insurance
- Access to a computer with internet and email (provided if needed)
- Cell phone for communication
- Strong time management skills
- Effective communication skills with the ability to empathize and connect with individuals
- Responsive communication with the agency and supervisors
