Social worker intern

Independence Rehab and Nursing a long-term care facility in Philadelphia, is seeking a social worker intern to join the social work team. They will work under direct leadership of the director of social services. We are looking forward to welcoming you to the team!

Education Requirements

• Must be currently enrolled in a Bachelor of Social Work (BSW) or Master of Social Work (MSW) program.
• The program should be accredited by the Council on Social Work Education (CSWE).
• Internship usually fulfills the student’s required field practicum hours.
• The intern must be supervised by SSD
• The intern cannot independently perform required social service functions that must be completed by the facility’s designated social worker.

• Criminal background check
• FBI Fingerprint clearance (if applicable)
• TB screening and health clearance

Typical Duties in a SNF

Under supervision, a social worker intern may:

• Assist with resident psychosocial assessments
• Participate in care plan meetings
• Help with discharge planning
• Provide resident and family support
• Assist with room changes, adjustment counseling, and grievance follow up
• Help connect residents with community resources
• Document progress notes or social service interactions
