Key Responsibilities:
- Coordinate and monitor individualized care plans
- Connect clients with appropriate community resources and services
- Advocate for client needs while promoting dignity and independence
- Maintains and promotes client satisfaction
- Provides leadership and oversight to direct and indirect care team members, ensuring productivity and performance align with agency policies and procedures
- Participate in and assist with the on-call schedule as needed
- Competently and accurately completes assigned tasks within established guidelines and time frames according to Agency policy and procedure.
- Perform additional duties as assigned by the immediate supervisor
- Experience in case management, social services, or healthcare preferred
- Dependable, punctual, and self-motivated
- Strong communication and organizational skills
