Position Summary

The Direct Support Professional II (DSP II) is an experienced provider of person-centered support to individuals with intellectual and developmental disabilities while serving as an experienced team member and role model for quality service delivery. DSP II promotes health, safety, independence, community inclusion, and meaningful outcomes by exercising sound judgment, modeling best practices, and reinforcing compliance with organizational policies and regulatory requirements.

Essential Job Duties and Responsibilities[MB1] :

Person-Centered Support

 

  • Provide person-centered services that promote choice, dignity, independence, and community inclusion.
  • Support the implementation of Individual Service Plans (ISPs), Behavior Support Plans, and risk mitigation strategies.
  • Teach and assist individuals with activities of daily living (ADLs), including cooking, cleaning, personal hygiene, money management, and other life skills that promote greater independence.
  • Provide one-to-one supports for individuals authorized to receive individualized services in accordance with approved service plans.
  • Encourage participation in community activities that reflect each individual's interests, preferences, goals, and cultural background.

Health, Safety & Medical Support

  • Assist individuals with activities of daily living, including bathing, dressing, eating, toileting, and personal care.
  • Administer medications and provide medication-related supports as authorized and in accordance with applicable regulations and organizational policies.
  • Monitor individuals' physical, behavioral, and emotional well-being, reporting concerns promptly and accurately.
  • Follow physician orders, treatment plans, medication protocols, and clinical recommendations.
  • Transport and accompany individuals to medical appointments as assigned, ensuring all follow-up instructions, medication changes, and physician recommendations are accurately documented and communicated to appropriate team members within 24 hours.
  • Recognize potential health and safety concerns and take appropriate action to reduce risks while maintaining individual dignity and independence.

Community Living & Daily Home Operations

  • Maintain a safe, clean, and comfortable home environment that supports individual health, safety, and quality of life.
  • Assist with meal planning, grocery shopping, food preparation, and cleanup while ensuring meals meet nutritional guidelines and individual dietary requirements.
  • Provide safe transportation using organization-owned vehicles for community activities, employment, appointments, and other approved services.
  • Support individuals in developing meaningful relationships and participating fully in their communities.

Documentation, Compliance & Quality Assurance

  • Complete required documentation, including Therap documentation, shift notes, medication records, incident reports, and other required documentation accurately and within established timeframes.
  • Consistently maintain documentation quality by achieving a minimum score of 85% on documentation audits.
  • Model best practices in documentation, professionalism, confidentiality, and regulatory compliance.
  • Assist in maintaining compliance with DBHDD, CARF, Medicaid, and InCommunity policies and standards.
  • Follow emergency procedures, complete required drills, and ensure all related documentation is completed accurately and timely.

Leadership, Collaboration & Professional Practice

  • Serve as an experienced resource for DSP (Uncertified) and DSP I staff by providing guidance, sharing best practices, and promoting quality service delivery.
  • Demonstrate professionalism, accountability, and person-centered practices through daily interactions with individuals served, families, coworkers, and community partners.
  • Collaborate effectively with Community Area Managers, clinicians, nurses, and interdisciplinary team members to ensure coordinated, high-quality supports.
  • Assist with onboarding and on-the-job coaching of new staff as requested by leadership.
  • Identify opportunities to improve service quality and communicate concerns or recommendations to leadership.
  • Participate actively in team meetings, required training, quality improvement initiatives, and professional development activities.

 

Qualifications:

Education, Certifications & Experience Requirements:

  • High school diploma or GED required.
  • Two years of direct support experience required, with a preference for three to five years’ experience.
  • Current valid driver's license and evidence of personal liability motor vehicle insurance coverage; the ability to meet InCommunity's driving standards.  
  • Current CPR/First Aid and Medication Administration certification required, or the ability to obtain certifications within the timeframe established by InCommunity.
  • CPR/First Aid and Medication Administration certification (or ability to obtain),
  • DSP II Certification through the University of Georgia, The National Association of Direct Support Professionals (NADSP) or another InCommunity-approved accredited program required, obtained within the timeframe established by InCommunity.

Supervisory Responsibilities:

This position does not have formal supervisory responsibilities but provides guidance and mentoring to DSP (Uncertified) and DSP I staff.

Work Environment & Physical Demands:

  • Residential and community settings with moderate noise.
  • Frequent standing, walking, bending, lifting up to 50 pounds, and assisting with transfers as needed.
  • Reasonable accommodations may be made to enable qualified individuals with disabilities to perform essential functions.

Travel Required:

Frequent local travel between residential homes, medical appointments, and community activities in an organization-owned vehicle, which is typically a mini-van or similar vehicle.
