You will ensure high-quality, efficient outcomes by collaborating with healthcare teams, patients, families, and payors. This role requires leadership in communication and problem-solving to meet clinical and organizational goals. What you will do: Lead team meetings and coordinate care plans, resolving treatment discrepancies Monitor patient progress and adjust care plans to optimize outcomes Ensure compliance with laws, safety regulations, and organizational values Facilitate timely discharge planning across the continuum of care Manage resources effectively, focusing on cost, quality, and length of stay What you will need: BSN preferred with RN Indiana licensure required 3-5 years clinical nursing experience, critical care highly desirable Certification in case management preferred Strong critical thinking and communication skills Computer proficiency; Spanish language skills a plus Our Commitment Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
