Overview
We are seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our team full-time. In this role, you will act as the clinical lead for your patients, performing initial assessments and managing the ongoing care plan to ensure optimal outcomes in an outpatient/home setting.
Compensation & Benefits
- Annual Salary: $95,000
- Schedule: Full-Time (FT)

- Support: Direct communication and daily updates from leadership to support your field operations.
Key Responsibilities
- Clinical Assessments: Perform initial assessments and evaluations for home care assistance in an outpatient setting.
- Care Planning: Collaborate with an interdisciplinary healthcare team to develop and implement comprehensive patient care plans.
- Documentation: Maintain precise and timely documentation of patient needs and progress in compliance with regulations.
- Advocacy: Serve as the primary point of contact for patients and their families, providing education and coordinating resources.
Qualifications
- Licensure: Must hold a valid, unrestricted Registered Nurse (RN) license.
- Experience: Previous experience in Home Care, Nursing Homes, or Assisted Living is strongly preferred.
- Clinical Expertise: Strong knowledge of nursing principles and proficiency in patient assessment.
- Soft Skills: Excellent critical thinking, time management, and the ability to communicate effectively with a diverse caregiving team.

In Home Personal Services began in January 2004, founded by Michael A. Collura working with his own Mother to provide quality caregiving services to seniors who wished to remain in the comfort of their own home.
We understood early on the challenges facing many seniors and their families when it came to aging and the desire to maintain a safe and independent lifestyle in the comfort of one’s familiar surroundings.
In Home Personal Service was formed with the belief that home care should be tailored to meet the individual needs of every client. It has remained our mission, to ensure the client and their family receive the highest level of quality care.
Many challenges were faced along the way including new laws and regulations for standards of quality care introduced by State authorities in 2008. Michael and the IHPS team were already operating well above these new standards, proving their mission was a success as they had not only met the standards of quality care issued by the regulatory authorities; they had exceeded them.
Today, Michael continues his mission and In Home Personal Services has grown not just in its native State of Illinois, but throughout the United States.
