Overview

We are seeking a proactive and compassionate Registered Nurse (RN) Case Manager to join our team full-time. In this role, you will act as the clinical lead for your patients, performing initial assessments, completing OASIS assessments, and managing ongoing care plans to ensure optimal patient outcomes in a home health setting.

Key Responsibilities

OASIS & Clinical Assessments: Complete OASIS assessments, Start of Care (SOC), recertifications, and patient evaluations in compliance with home health regulations.

Care Planning: Collaborate with an interdisciplinary healthcare team to develop and implement individualized patient care plans.

Documentation: Maintain accurate and timely clinical documentation, including OASIS documentation, patient progress, and care coordination records.

Advocacy: Serve as the primary point of contact for patients and their families, providing education and coordinating resources.

Qualifications

Licensure: Must hold a valid, unrestricted Registered Nurse (RN) license.

Experience: Previous experience in Home Health, including OASIS assessment experience, is strongly preferred. Experience in nursing homes, assisted living, or related healthcare settings is also valued.

Clinical Expertise: Strong knowledge of nursing principles, patient assessment, and care management.

Soft Skills: Excellent critical thinking, time management, and ability to communicate effectively with patients, families, and the caregiving team.
