Job SummaryThe Licensed Practical Nurse (LPN) in Long-Term Care provides supportive and restorative nursing care to assigned residents under the direction of a Registered Professional Nurse (RN). The LPN functions as an essential member of the nursing team and the interdisciplinary care team to enhance resident well-being, preserve safety, and support an improved quality of life. Nursing care is delivered in accordance with facility policies, procedures, and Person-Centered Care planning practices. Required Skills & Experience· Completion of an accredited LPN/LVN Nursing Program· Demonstrated clinical competency within the scope of practical nursing· Strong ability to form positive relationships with residents, families, and internal teams· Ability to inspire teamwork and uphold high staff morale· Experience leading and directing CNA-delivered care· Ability to meet physical and medical care demands of long-term care residents with complex conditions· 1 years of relevant long-term care nursing experience Licensure & CertificationsRequired· Licensed and currently registered LPN in New York State· BLS (American Heart Association) or NY State CPR Certification· Must be local to Brooklyn / surrounding areas· Able to work every other weekend
