Primary ResponsibilitiesDirect Client ServicesConduct comprehensive intakes and provide accurate information, referrals, and resources.Screen and assess client needs, including entitlements, benefits, and eligibility for emergency cash assistance.Advocate on behalf of clients to ensure timely access to services and benefits.Develop, implement, and monitor individualized care plans.Provide individual supportive counseling and crisis intervention as needed.Facilitate support groups for children of Holocaust survivors.Case Management & DocumentationMaintain complete, accurate, and timely case notes in accordance with organizational standards.Prepare all required documentation within established timeframes.Track and report service statistics and outcomes using program data systems.Conduct home visits throughout the five boroughs as required.Community Outreach & CollaborationConduct outreach to Holocaust Survivor Program (HSP) providers and other NYC organizations serving survivors and their families.Build and maintain collaborative relationships with community organizations, government agencies, and service providers.Participate in supervision, staff meetings, and interdisciplinary team discussions.Student Supervision & Program SupportSupervise social work students, when applicable.Support program initiatives, special projects, and community-based activities.Demonstrate positive interpersonal relationships with colleagues and uphold organizational culture.Compliance & Professional StandardsComplete all mandatory annual trainings, including:Sexual Harassment PreventionHIPAAOSHAIT Security AwarenessOther required compliance trainingsAdhere to Selfhelp policies related to attendance, professional conduct, safety, and ethical standards.Maintain a high level of customer service and treat all clients with dignity and respect.Qualifications & Job CompetenciesRequiredLMSW – Active New York State licenseMinimum of 2 years of experience working with older adults (geriatrics)Ability and willingness to travel throughout the five boroughsProficiency in Microsoft Office Suite and comfort with technologyStrong communication, listening, and advocacy skillsExcellent customer service with a client-centered approachAbility to multitask, work independently, and manage a detailed caseloadHigh attention to detail with strong documentation skillsPreferred3–5 years of experience working with older adults and/or traumatized populationsExperience working with Holocaust survivors, their families, or trauma-informed populationsFamiliarity with NYC entitlement systems, benefits programs, emergency assistance, and social service networksWorking Conditions / Physical DemandsOffice-based environment with regular computer and phone useRegular travel via public transportation for home visits and community outreachAbility to work remotely 2–3 days per week based on program needs
Salary : $65,000 - $70,000
