Substance use case managers support clients in identifying all relapse prevention through connection to community resources and supportive services. Case managers must be able to build a healthy rapport with clients, maintain a large caseload, and prepare/carry out Individual Service Plans with individuals receiving services. The ability to complete weekly documentation, monthly progress reports, and initial authorization screenings for various insurance carriers is essential.
DUTIES
- Support clients in all relapse prevention through connection to community resources and supportive services.
- Assist with care coordination to ensure all vendors are working in collaboration enabling the greatest opportunity for client success.
- Arrange and attend all vender meetings
- Coordinate and support client to attend Substance Use prevention groups, medical/psychological appointments
- Report changes concerning needs and/or appropriateness for services.
- Prepare and carry out Individual Service Plans, quarterly reports, discharge summaries, daily progress notes, and any additional documentation with individuals receiving services.
- Document case activity in accordance with program policy and maintain case records in compliance with Agency, regulatory, and accreditation standards.
- Assist clients through linking to community resources and developing support networks within the community.
- Have a flexible schedule and be on call if necessary
- Participate in supervision with the Program Director and/or Clinical Supervisor as required
QUALIFICATIONS
- Must have 1 year of case management experience working with individuals with mental health and/or substance use issues
- Must have a valid Virginia driver's license
- Must have a Bachelor's Degree in Social Work, Education, Sociology, Psychology or a degree related to the Human Services field
- MUST ALREADY HAVE A QMHP CERTIFICATION
