PRN - Hospice Admission Nurse

About the Role:

The PRN Hospice Admission Nurse plays a critical role in providing compassionate and comprehensive care to patients and their families during the hospice admission process. This position involves conducting thorough patient assessments to determine eligibility and appropriate care plans in alignment with hospice philosophy. The nurse serves as a vital liaison between patients, families, physicians, and interdisciplinary teams to ensure seamless transitions into hospice care. Minimum Qualifications:

• Current and valid Registered Nurse (RN) license in the state of Oregon.
• Strong knowledge of hospice care principles, symptom management, and end-of-life care.
• Ability to perform comprehensive patient assessments and develop care plans.
• Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare teams.

Preferred Qualifications:

• Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
• Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related fields.
• Experience working in home health or hospice admission nursing roles.
• Familiarity with electronic medical records (EMR) systems and documentation standards.
• Training in cultural competency and sensitivity related to end-of-life care.
• Ability to work independently with strong organizational and time management skills.
