At Primrose Hospice Care, we believe in bringing hospice back to its heart — compassionate, personal, and patient-centered. As a Hospice Registered Nurse (RN), you’ll provide skilled nursing care and emotional support to patients and families wherever they call home.
This role combines clinical expertise with compassion, ensuring comfort, dignity, and peace through every stage of care — while maintaining compliance with CMS Conditions of Participation (42 CFR §418) and Oregon state hospice regulations.
What You’ll Do
- Perform comprehensive assessments and ongoing evaluations of patient and family needs
- Develop and update individualized Plans of Care with the Interdisciplinary Group (IDG)
- Provide direct patient care, symptom management, and medication administration
- Communicate promptly with physicians, team members, and family caregivers
- Educate patients and families on hospice philosophy, symptom control, and the dying process
- Maintain accurate, timely documentation per CMS and agency standards
- Participate in IDG meetings to coordinate care and review outcomes
- Support CNAs, LPNs, and other team members with leadership and collaboration
- Uphold infection control, safety, and compliance protocols
- Provide comfort, emotional support, and respect for each patient’s values and wishes
What You'll Bring
- Graduate of an accredited school of nursing
- Current Registered Nurse (RN) license in Oregon
- Valid driver’s license, reliable vehicle, and proof of insurance
- Minimum 1 year of nursing experience (hospice, home health, or acute care preferred)
- Compassionate communication skills and ability to work independently
- Knowledge of hospice philosophy, palliative care, and CMS hospice regulations
