Your primary goal is to ensure eligible individuals receive comprehensive information about our program and seamlessly transition into our care.
What You Will Do
- Serve as the initial point of contact for newly qualified referrals, providing an overview of the PACE model and assessing eligibility.
- Follow up promptly on qualified referrals, documenting activities accurately in our systems.
- Collaborate with the enrollment team to manage referrals through the entire enrollment process.
- Promote the PACE program to potential referrals, their families, and community agencies through presentations and events.
- Coordinate enrollment assessments, maintain documentation for tracking and reporting purposes, and assist with Medicaid application processes.
- Communicate regularly with interdisciplinary team members and referral sources to ensure a smooth enrollment process.
- Associate's Degree preferred, with at least two years of sales or marketing experience.
- Valid driver's license, proof of car insurance, and reliable transportation required.
- Demonstrated ability to communicate effectively, both verbally and in writing.
- Proficiency with Microsoft Office suite and other necessary software.
- Strong organizational skills and the ability to prioritize workload effectively.
