- Guide participants and families through Medicaid applications, approvals, and re-determinations.
- Conduct home visits to complete applications and gather required information.
- Collaborate with county agencies, participants, caregivers, and internal teams to ensure eligibility requirements are met.
- Educate families on Medicaid rules and regulations, including Long-Term Care Medicaid, trusts, and spousal impoverishment provisions.
- Monitor and track eligibility status, maintaining accurate records and documentation.
- Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams.
- Maintain strict confidentiality and compliance with all federal, state, and organizational requirements.
- High school diploma or equivalent required.
- Comprehensive knowledge of Medicaid programs, eligibility requirements, and benefits.
- Understanding of intake practices and geriatric assessment principles.
- Knowledge of community resources available to seniors and their families.
- Minimum of one (1) year of experience working with frail, elderly, or aging populations.
- Strong communication, organizational, and customer service skills.
- Ability to work independently while building collaborative relationships with participants, families, and team members.
