Employment Type: Full time Shift: Description: Medicaid Eligibility Specialist Location - St Joseph PACE Shift - 1st Shift Hours - 8-4:30 Monday - Friday In person - No Remote We are seeking a compassionate and detail-oriented Medicaid Eligibility Specialist to support older adults and their caregivers through the Medicaid application and eligibility process. In this role, you will serve as a trusted resource, helping participants navigate Medicaid requirements while ensuring timely and accurate submission of applications, re-determinations, and supporting documentation. Key Responsibilities Guide participants and families through Medicaid applications, approvals, and re-determinations. Conduct home visits to complete applications and gather required information. Collaborate with county agencies, participants, caregivers, and internal teams to ensure eligibility requirements are met. Educate families on Medicaid rules and regulations, including Long-Term Care Medicaid, trusts, and spousal impoverishment provisions. Monitor and track eligibility status, maintaining accurate records and documentation. Complete HMO disenrollment paperwork and communicate progress to enrollment and leadership teams. Maintain strict confidentiality and compliance with all federal, state, and organizational requirements. Qualifications High school diploma or equivalent required. Comprehensive knowledge of Medicaid programs, eligibility requirements, and benefits. Understanding of intake practices and geriatric assessment principles. Knowledge of community resources available to seniors and their families. Minimum of one (1) year of experience working with frail, elderly, or aging populations. Strong communication, organizational, and customer service skills. Ability to work independently while building collaborative relationships with participants, families, and team members.
