Ambulatory Clinical Pharmacist – Anticoagulation Clinic

Location: Troy, Albany, and Clifton Park, NY

Status: Full-Time

Expected Weekly Hours: 75 Hours Bi-Weekly

Shift: Day Shift, Monday–Friday

Position Summary

The Ambulatory Clinical Pharmacist is responsible for providing comprehensive anticoagulation management services within a collaborative, multidisciplinary care model. This role works directly with patients, providers, and healthcare teams to optimize anticoagulation therapy, improve clinical outcomes, enhance medication safety, and support value-based care initiatives.

Key Responsibilities

Anticoagulation Clinic Management

• Provide clinical oversight and guidance for Anticoagulation Clinic services.
• Manage patients receiving anticoagulation therapy utilizing evidence-based protocols and best practices.
• Review medication regimens, laboratory values, and patient-specific factors to optimize anticoagulation outcomes.
• Adjust anticoagulation therapy as appropriate under approved Collaborative Drug Therapy Management (CDTM) agreements.
• Prescribe medications within the scope of provider-approved CDTM protocols.
• Monitor patients for therapeutic effectiveness, adverse events, and safety concerns.
  
  

Patient Care and Education

• Assess, document, and communicate patient progress and response to therapy.
• Educate patients and caregivers regarding anticoagulation medications, monitoring requirements, adherence strategies, and lifestyle considerations.
• Respond to patient inquiries and triage clinical concerns via telephone and telehealth platforms.
• Serve as an advocate for patient-centered anticoagulation management.
  
  

Care Coordination

• Collaborate effectively with physicians, advanced practice providers, nurses, and other healthcare professionals.
• Communicate recommendations, interventions, and outcomes to referring providers.
• Coordinate patient scheduling and maintain efficient clinic workflow.
• Ensure timely patient access, especially for urgent clinical needs.
  
  

Documentation and Quality Improvement

• Maintain accurate, organized, and timely documentation of clinical interventions, recommendations, follow-up plans, and outcomes.
• Participate in departmental quality improvement, performance improvement, and research initiatives.
• Assist with quality assurance reporting and regulatory readiness activities.
• Support preparation for accreditation, regulatory inspections, and compliance reviews.
  
  

Education and Professional Development

• Participate in the training and development of PGY1 Pharmacy Residents and PharmD students.
• Maintain competency in ambulatory care and anticoagulation management through ongoing professional development.
• Participate in departmental, multidisciplinary, hospital, and health-system committees as a leader, member, or consultant.
  
  

Population Health and Value-Based Care

Support Organizational Value-based Care Initiatives, Including

• Medication adherence programs for:
• Diabetes medications
• Renin-Angiotensin System (RAS) antihypertensives
• Statin therapies
• Statin Use in Persons with Diabetes (SUPD)
• Comprehensive Medication Reviews (CMRs)
• Targeted medication interventions
• Cost-savings initiatives
• Referral-based medication management services
• Pharmacotherapy consultations focused on effectiveness, safety, and value
  

Minimum Qualifications

• Current New York State Pharmacist License (by examination or reciprocity).
• Demonstrated competence in providing direct patient care.
• Strong clinical assessment and communication skills.
• Ability to work effectively in a collaborative healthcare environment.
  
  

Preferred Qualifications

• PGY1 and/or PGY2 Residency Training in Ambulatory Care, or equivalent fellowship training.
• Five (5) or more years of ambulatory care clinical pharmacy experience.
• Board Certified Ambulatory Care Pharmacist (BCACP), or eligible within one year of hire.
• Meets New York State requirements for Collaborative Drug Therapy Management (CDTM).
• Previous anticoagulation management experience preferred.
• Experience working in telehealth and ambulatory care settings.
• Knowledge of healthcare reimbursement and insurance processes.
• Excellent communication, interpersonal, organizational, and time-management skills.
• Self-motivated and detail-oriented with the ability to work independently.

Pay Range: $56.00 - $72.30

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
