Nurse Practitioner - Spanish Required

We are seeking a Nurse Practitioner to provide high-quality, value-based primary care for adult Medicare patients in an outpatient setting.

• Independently conduct office visits for routine and acute issues, including wellness exams and health assessments.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care and coordinate with specialists and facilities.
• Lead population health initiatives, promote patient education, and assist with outreach and home visits as needed.
• Build trusting relationships with patients to support long-term health and well-being.
  
  

Qualifications include: MS or DNP, national certification (FNP, AGNP, ANP, GNP), active NP license, DEA license, and US work authorization. Preferred experience in primary care or geriatrics, with knowledge of coding and risk assessments.
