Case Manager - Transitional Housing
The case management position requires patience and a balanced, professional outlook. The case manager will exhibit ICNA Relief USA’s values and serve as a positive role model for Transitional Housing clients. (SEE: Minimum Case Management Standards)
• Provides instruction and proper guidance to clients.
• Conducts client intake in collaboration with Site Coordinator.
• Develops Individual Service Plan as required by the program.
• Identifies benefits or entitlements that may be available to the client.
• Meets with the clients as required, to evaluate progress and assist client in developing and meeting individual goals.
• Research and have familiarity with local area services related to health and human services, developing working relationships and collaborations.
• Provides the necessary referrals for services, including mental health and/ or substance abuse counseling.
• Advocates on the client’s behalf when necessary.
• Maintains data collection and case records as required by applicable rules and regulations.
• Helps clients regain and maintain control over their lives and help them transition to independent living.
• Facilitates development of self-help skills, support systems, coping strategies, and increased hope and self-esteem that are central to the process.
• Ensures that clients adhere to the program rules and regulations and other duties as assigned by the Program Director or designee.
• Ensure client meets or exceeds personal action plan goals.
• Meet or exceed HIPAA regulations by law.
• Any other role or responsibility assigned by Site Coordinator.
NECESSARY SKILLS:
• Experience providing case management services, including developing an individual service plan for at risk populations.
• Understands the challenges facing homeless populations.
• Consistently maintain required case records.
• Ability to develop and maintain appropriate boundaries.
• Must demonstrate sensitivity to needs of clients in crisis.
• Must demonstrate responsiveness to cultural differences and a commitment to the value of cultural competency.
Essential Duties and Responsibilities: The person filling this position is expected, under general supervision, to:
(1) immediately formulate a plan and prepare clients for permanent housing placement.
(2) engage individuals requiring social service assistance.
(3) interview and evaluate clients, formulate Independent Action Plans and goals; and
(4) locate and refer to appropriate community resources for clients, including the following:
• Serve as a liaison with outside organizations regarding such matters as employment, education, healthcare, housing, social services, legal issues, etc.
• Advise clients in dealing with issues as: behavior, educational progress, family dynamics, mental and physical health, housing, personal finances, substance abuse, mental and/or physical trauma.
• Consult with others to determine causes of client problems and effective solutions.
• Conduct initial intake and assessment of clients' needs and periodic re-assessments.
• Create and maintain client electronic and paper files.
• Obtain and maintain relevant documentation and release forms from clients (i.e., birth certificate, proof of income, medical documents, release of Personal Health Information, etc.).
• In collaboration with clients, conduct periodic revisions of independent action plans including short term and long-term client goals.
• Assist clients in attaining their goals by identifying community resources and by referring clients to appropriate services.
• Work with clients to break through barriers to their goal(s) attainment.
• Assist clients in advocating for themselves in order to gain housing stability.
• Monitor clients' progress toward their goals via regularly scheduled face-to-face contacts/sessions.
• Document all client encounters via case management notes
• Follow-up with clients with referral organizations regarding client contact and progress with referral organization.
• Provide all required information for weekly/monthly/quarterly/annual reports.
MINIMUM EDUCATION/EXPERIENCE REQUIRED: Master's Degree or related field and/or equivalent experience.
OTHER REQUIREMENTS: May be required to become First Aid/CPR certified. Bi-lingual preferred; English/Arabic - English/Urdu
