Position Summary
iCare Health Services LLC is seeking a reliable and compassionate HCBS Provider to provide Attendant Care services to a member in the Flagstaff area. The provider will assist with mobility support and daily living activities while promoting independence and safety within the home and community.
This is a 1099 contractor position with a consistent weekly schedule.
Schedule
- Total Hours: 6 hours per week
- Days: Mondays and Thursdays
- Hours: 3 hours each day
- Start Time: 10:00 AM
Pay
- $16.00 per hour
- 1099 Independent Contractor position
Essential Duties and Responsibilities
- Provide Attendant Care services in accordance with the member’s Individual Support Plan (ISP)
- Assist with mobility and daily living activities
- Support high-functioning daily tasks to promote independence
- Provide supervision to ensure the member’s safety and well-being
- Assist with personal care, hygiene, and household tasks as needed
- Use Electronic Visit Verification (EVV) to clock in and out of services
- Communicate effectively with supervisors and family members regarding the member’s needs
- Maintain accurate documentation of services provided
- Follow all company policies, procedures, and safety standards
- Report incidents or concerns promptly to the HCBS Supervisor
Requirements
- Must be 18 years of age or older
- High School Diploma or GED
- Experience working with individuals with developmental or physical disabilities preferred
- Must have a valid Arizona driver’s license
- Must have reliable transportation
- Must be able to assist with mobility support
- Must be able to lift up to 40 pounds
- Must be dependable and able to work independently
- Must be able to pass background and fingerprint clearance requirements
