What are the responsibilities and job description for the Behavioral Program Director position at iCare Health Network?
The primary role of the Facility Behavioral Health Director is to coordinate, supervise and facilitate the delivery of Resident Behavioral Health Services to facility residents.
Responsibilities:
Direct the facilities Behavioral Health Program (BHP) consistent with Behavioral Health Program operational guidelines.
Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed.
Works in collaboration with BHP participating residents and the interdisciplinary treatment team in developing a Recovery-based plan of care and services for residents.
Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors.
Serves as a role model to all Facility employees, families and residents; maintains a positive and respectful attitude to all peers and staff.
Qualifications:
Master’s Degree in Social Work (MSW), preferred.
Bachelor’s Degree in Social work or related human services field, required.
A minimum of one years’ experience in long term care providing direct social services, required.
A minimum of one years’ experience providing social services to behavioral health clients, required.
