Licensed Practical Nurse (LPN)

Position Overview

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a professional healthcare setting. The LPN will administer medications and treatments, monitor resident conditions, maintain accurate documentation, and collaborate with residents, families, and members of the interdisciplinary care team to ensure safe and effective care.

Schedule

  • Full-Time or PRN opportunities available
  • 12-hour shifts: 6:00 AM – 6:00 PM
  • Rotating block schedule
  • Two consecutive days off
  • Every other weekend

Key Responsibilities

  • Administer medications and perform treatments as ordered by the physician and within the scope of the LPN license.
  • Provide medications through various routes, including oral, enteral, parenteral, and suppository administration.
  • Perform and assist with a variety of treatments, including wound care, sterile and aseptic procedures, postural drainage and percussion, tracheostomy care, gastrostomy and jejunostomy care, and catheter care.
  • Observe residents for changes in condition and evaluate and report the effectiveness of medications and treatments.
  • Ensure adequate supplies, equipment, and medications are available for resident care.
  • Receive, document, and communicate information regarding residents' medical, social, functional, and other care needs.
  • Participate in receiving and providing shift reports to ensure continuity of care.
  • Communicate effectively with residents, families, healthcare professionals, and other staff members.
  • Complete initial nursing assessments for newly admitted residents.
  • Assist with the development and completion of Minimum Data Set (MDS) assessments, quarterly reviews, and individualized care plans.
  • Maintain accurate and timely clinical and administrative documentation, including diagnoses, observations, treatments, and resident progress.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Attend and actively participate in staff meetings, training, and other required activities.
  • Follow established policies, procedures, and applicable nursing regulations and standards.
  • Respond appropriately to medical emergencies and changes in residents' conditions.
  • Work collaboratively with the interdisciplinary team to promote resident safety, comfort, and quality of life.

Knowledge & Qualifications

  • Knowledge of gerontological and geriatric nursing principles and practices.
  • Understanding of the effects of the aging process.
  • Knowledge of team nursing practices and procedures.
  • Knowledge of treatment plans and appropriate nursing interventions.
  • Knowledge of pharmacology, medication administration, and medication safety.
  • Understanding of applicable federal, state, and organizational laws, regulations, and standards governing practical nursing practice.
  • Knowledge of proper procedures for the security, control, storage, and administration of medications and medical supplies.
  • Current Licensed Practical Nurse (LPN) license in good standing.
